Changed the injection of the JavaScript to use middleware, rather than using the BOTTOM_BANNER and ConfigRevisions

NOTE: If you used v1.4.0 - 1.4.3, then you have to remove the BANNER_BOTTOM config from the configuration.py or the ConfigRevision
