blur sensitive images procedure

current templates have "blur" class to hide sensitive thumbnails on features and item pages. Rather than using a class to do this, set up procedure to actually blur the small and thumbnail image itself, or use a standard placeholder image in the metadata rather than actual thumb.

guidelines:

• consider if the collection really needs sensitive/graphic images in the accessible content
• if graphic images are necessary in the collection, do not fill in image_thumb or image_small with actual images. Use a placeholder image instead
• use the "sensitive" column in the metadata to provide a warning, add to configured metadata displays
