As a user, I want to be able to record whether or not a client has attended a service. #56

This part of the form should conditionally render depending on whether the client has any current referrals
I should see a the name of the client
I should see selection of tickboxes with the names of the current client referrals on them
I should be able to select the ones the client has attended
I should see a text box where I can fill in their reasons for not attending
I should see a “next” button which will take me to the service referrals page
