Unify with application/problem+json

Mark Nottingham has submitted a similar type in draft-nottingham-http-problem. Compare the types and see if there can be a unified type.
E.g. Nottingham's type has no defined way to add hyperlinks which the client can use to resolve the problem. His type attribute might also be a profile link.
