domain reports including all clinical domains (conditions, drug, observation, procedure, etc...)

this requires addition of the sex proportions to the concept level file export. Currently it only is exported in the stratification by age and sex. To be clear, this should be applied to the individual concept reports, not the overall domain level reports.
